Forma Med is an AI-powered platform designed to assist users in navigating the medical school admissions process. The Service may include features such as application review, essay feedback, strategic guidance, and educational resources. The Service uses artificial intelligence, including third-party AI services, to process and analyze user-submitted content.
You retain ownership of all content you submit to the Service, including personal statements, application materials, and other information ("User Content"). By submitting User Content, you grant us a non-exclusive, worldwide, royalty-free license to use, process, store, and analyze your User Content solely for the purpose of providing and improving the Service.
You agree not to submit any content that is unlawful, defamatory, fraudulent, or infringes on the rights of others. You may not use the Service to generate false or misleading application materials.
The Service provides AI-generated feedback, suggestions, and guidance. This content is provided for informational and educational purposes only. AI-generated content does not constitute professional advice and should not be relied upon as a substitute for independent research or consultation with qualified professionals. We do not guarantee the accuracy, completeness, or suitability of AI-generated content for any particular purpose. You are solely responsible for all decisions you make based on the Service.
Certain features of the Service may require payment. All fees are stated in U.S. dollars unless otherwise specified. You agree to pay all applicable fees in accordance with the pricing and payment terms presented to you at the time of purchase. All payments are processed through our third-party payment processor. Fees are non-refundable except as expressly stated in these Terms or required by law.
The Service and its original content, features, and functionality (excluding User Content) are and will remain the exclusive property of Change Systems Inc. and its licensors. The Service is protected by copyright, trademark, and other laws of the United States and foreign countries. Our trademarks may not be used in connection with any product or service without our prior written consent.

WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, SECURE, OR ERROR-FREE. WE DO NOT GUARANTEE ANY PARTICULAR OUTCOME FROM YOUR USE OF THE SERVICE, INCLUDING ADMISSION TO ANY MEDICAL SCHOOL OR EDUCATIONAL PROGRAM.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT SHALL CHANGE SYSTEMS INC., ITS DIRECTORS, OFFICERS, EMPLOYEES, AGENTS, OR AFFILIATES BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS OR REVENUES, WHETHER INCURRED DIRECTLY OR INDIRECTLY, OR ANY LOSS OF DATA, USE, GOODWILL, OR OTHER INTANGIBLE LOSSES, RESULTING FROM: (A) YOUR USE OR INABILITY TO USE THE SERVICE; (B) ANY UNAUTHORIZED ACCESS TO OR USE OF OUR SERVERS AND/OR ANY PERSONAL INFORMATION STORED THEREIN; OR (C) ANY OTHER MATTER RELATING TO THE SERVICE. OUR TOTAL LIABILITY FOR ALL CLAIMS RELATED TO THE SERVICE SHALL NOT EXCEED THE GREATER OF ONE HUNDRED DOLLARS ($100) OR THE AMOUNTS YOU HAVE PAID TO US IN THE TWELVE MONTHS PRECEDING THE CLAIM.
You agree to defend, indemnify, and hold harmless Change Systems Inc. and its officers, directors, employees, and agents from and against any claims, liabilities, damages, losses, and expenses, including reasonable attorneys' fees, arising out of or in any way connected with: (a) your access to or use of the Service; (b) your User Content; or (c) your violation of these Terms.
We may terminate or suspend your account and access to the Service immediately, without prior notice or liability, for any reason, including if you breach these Terms. Upon termination, your right to use the Service will immediately cease. All provisions of these Terms which by their nature should survive termination shall survive, including ownership provisions, warranty disclaimers, indemnity, and limitations of liability.
We reserve the right to modify these Terms at any time. We will provide notice of material changes by posting the updated Terms on the Service and updating the "Effective Date" above. Your continued use of the Service after such changes constitutes your acceptance of the new Terms.
These Terms shall be governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of law provisions. Any dispute arising from or relating to these Terms or the Service shall be resolved exclusively in the state or federal courts located in Delaware, and you consent to the personal jurisdiction of such courts.
If any provision of these Terms is held to be unenforceable or invalid, such provision will be modified to the minimum extent necessary to make it enforceable, and the remaining provisions will continue in full force and effect.
These Terms, together with our Privacy Policy, constitute the entire agreement between you and Change Systems Inc. regarding the Service and supersede all prior agreements and understandings.
